What is sunken under-eye filler?

Some under-eyes are not just shadowed but genuinely sunken: the eye seems to sit deep in its socket, the lower lid looks hollow from the inner corner all the way to the outer edge, and the cheek beneath has flattened. This pattern is common in slim faces, after significant weight loss, in people with a naturally recessed orbital rim, and in later decades when the bone around the eye socket enlarges and the cheek fat descends. A single line of filler in the tear trough does not fix it, and simply adding more gel under the eye creates puffiness. Sunken under-eye filler takes a structural view instead.

The approach is layered. First, a firm hyaluronic acid gel such as Juvederm Voluma or Restylane Lyft is placed deep on the bone at the top of the cheek and along the orbital rim, rebuilding the shelf that the lower lid rests on. That alone usually lifts the floor of the hollow by half. Second, at the same visit or four weeks later, a small amount of soft, low-swelling gel is placed in the remaining groove to fine-tune. Splitting the work keeps volumes small, lets swelling settle before the final judgement, and avoids the overfilled look that under-eyes are notorious for.

What to expect

1. Consultation & diagnosis

We examine the depth and extent of the hollow, the cheek, the skin thickness and any fat bulge, and photograph you in standard lighting. We explain the two-layer plan, the products and volumes for each layer, and whether one visit or two is wiser for you.

2. Treatment

After numbing cream, the deep support layer is placed on the bone and checked seated. If appropriate, a small superficial refinement follows the same day; otherwise it is scheduled at four weeks. Each visit takes 30–40 minutes.

3. Aftercare & follow-up

Written aftercare goes into your care notebook. At four weeks we review swelling and symmetry and complete the second layer if planned. Maintenance is usually 12–18 months later.

• Avoid rubbing, eye massage, swimming goggles and tight glasses for 48 hours.
• Limit salt and alcohol for 48 hours to reduce water retention under the eye.
• Bruising from the deep layer can take 5–7 days to fade; arnica or a cool pack may help.
