Overview
What is calf Botox?
The gastrocnemius is the two-headed muscle that forms the rounded shape at the back of the lower leg. It can be naturally large or build up with heels, running, dancing or sport. When this muscle is the main source of calf width, relaxing part of it with Botox lets it gradually become smaller from reduced use, giving a slimmer, smoother outline.
Calf shape is not only muscle. Fat, fluid, bone and the deeper soleus muscle all contribute, and toxin only changes the muscle it reaches, so the first step is checking what your calves are actually made of. Calf Botox is a common request in Seoul, and many patients in Gangnam consider it alongside other body treatments.
How calf Botox works at Oaro
At Oaro Dermatology Clinic, where Apgujeong meets Cheongdam, Dr. Chang-hyun Kim or Dr. Ji-ho Park examines your calves while you stand and rise onto tiptoe, feeling how the inner and outer heads of the muscle contract. The dose and injection points are set by the dermatologist after examining muscle size and strength, and uneven legs are often treated slightly differently. If fat is the bigger factor, the doctor may suggest body contouring instead; our guide to Botox in Korea for foreigners covers the toxins used here.

Treatment goals
Common goals for calf Botox
A slimmer calf line in skirts and dresses
Many people want the lower leg to look less muscular from behind and from the side. As the muscle softens over 4–8 weeks, the calf curve usually becomes less pronounced, while most people find everyday walking feels the same.
Softening calves built up by sport or heels
Dancers, runners and people who wear heels for work often develop a firm bulge high on the inner calf. Botox can reduce this, although very active people may find the effect lasts a little less.
Balancing uneven calves
One calf is often larger than the other. Because each leg is assessed separately, the dose can be adjusted on each side and fine-tuned at the next session.
Planning ahead for summer or an event
Because the change is gradual, calf Botox is ideally planned about two months before a wedding or holiday. Treating close to the date may not leave enough time to see a difference.
Your visit
What to expect
1. Consultation & diagnosis
The dermatologist asks about exercise, footwear, previous toxin and your health, then examines both calves relaxed and on tiptoe. You will hear whether muscle is the main cause, what change is realistic, and the suggested toxin and dose, with an itemised quote in English.
2. Treatment
You lie face down while the skin is cleaned and marked, and small amounts of toxin are placed across the bulkiest part of each calf with a fine needle. Both legs take about 10–20 minutes; most people feel a series of quick stings, and ice can be used if you prefer.
3. Aftercare & follow-up
You can walk out straight away with written aftercare. A review at around 2–4 weeks checks symmetry and response, and if you have travelled home we follow up by message using photos.
Cost guide
Calf Botox cost in Seoul
Calves need considerably more toxin than facial areas, so they cost more. These are typical ranges in Seoul clinics, not fixed prices. Units are not interchangeable between brands, so comparing quotes by unit numbers alone can mislead; Korean-made toxins such as Nabota generally cost less than imported brands.
| Option | Typical range (KRW) |
|---|---|
| Both calves, Korean-made toxin (e.g. Nabota) | ₩150,000–₩400,000 |
| Both calves, larger or very strong muscle, Korean-made toxin | ₩300,000–₩600,000 |
| Both calves, imported toxin | ₩400,000–₩900,000 |
Exact pricing is confirmed at consultation after examination.
Aftercare
Aftercare
- Walk normally, but avoid intense calf workouts, long runs, saunas and calf massage for 1–2 days.
- Do not rub or press on the injected areas, and avoid alcohol, on the day of treatment.
- Small bruises or mild aching can occur; a cool compress helps, and bruises usually fade within a week or two.
- Calves may feel tired on stairs or hills for a few weeks, so allow extra time when sightseeing.
- Contact the clinic about unusual weakness or any concern, and seek urgent care for difficulty swallowing or breathing, which is very rare.

FAQ
Frequently asked questions
Will calf Botox affect my walking or exercise?
Everyday walking is usually unaffected, but many people notice calf fatigue on stairs, hills or when standing on tiptoe for a few weeks. Runners, hikers and dancers should mention this so treatment can be timed around training or competitions. Bruising, mild aching or swelling and, occasionally, headache can also occur.
Who should avoid or postpone calf Botox?
Postpone treatment during pregnancy or breastfeeding, and avoid it with neuromuscular disorders such as myasthenia gravis, an allergy to toxin ingredients, or infection or broken skin on the calves. Tell the doctor about your medicines, particularly aminoglycoside antibiotics and blood thinners, and any history of leg swelling, blood clots or vein problems.
I'm visiting Seoul. When should I book, and can I fly afterwards?
Book early in your trip so there is time for a check before you leave; most of the slimming will appear at home over 4–8 weeks, and we follow up by message. Flying is generally fine; on long flights, walk the aisle and move your ankles as usual. The clinic, on the Apgujeong and Cheongdam border, is about 8 minutes on foot from Apgujeong Rodeo Station and opens on Saturdays from 10:00 to 16:00.
How long does calf Botox last?
Usually about 4–6 months, after which the muscle gradually regains strength and some bulk. With repeat sessions the muscle often stays smaller for longer, so intervals can sometimes be extended, but the change is not permanent.
Can calf Botox reduce fat or swelling?
No. Toxin only relaxes muscle, so it will not change fat, fluid retention or bone shape. If your calves feel soft rather than firm, or swell by evening, the dermatologist at our Gangnam clinic will explain this honestly and may suggest other options or a medical check.
